Re: Immediate Action: Proof of Vaccination Due July 15

This note is a reminder that all students must be fully vaccinated against COVID-19 in order to come to campus in the fall.

In order to ensure that we have all the information prior to the start of school, we have set the deadline for submitting evidence of vaccination to be July 15, 2021. Please review the instructions for how to upload your information into Patient Gateway for Health Services, provided by Newton Wellesley Collegiate Health. You must also provide vaccination information in the toolbox on the Wellesley portal.

Please upload a picture of your vaccination card, or, if you have not already done so, immediately make an appointment to get a vaccine. Students who have not submitted proof of vaccination or received approval of a medical or religious exemption or disability accommodation by July 15 will not be allowed to return to campus. If you are unable to access a vaccine, please contact healthmd@wellesley.edu to discuss whether you can be vaccinated upon arrival to campus. If you have already uploaded evidence of vaccination, you may disregard this message.
