Re: Important Information Regarding On-Campus Flu Shot Clinics and COVID Boosters

This fall, we will be offering opportunities for faculty and staff to receive COVID booster shots and flu shots on campus. We encourage all members of our community to get both of these shots.
COVID-19 bivalent booster shots will be available on Monday, October 17, from 9 a.m. to 3 p.m. in Tishman Commons. You can pre-register for a booster shot here.
We can issue vaccination cards for the boosters, and we can document the booster on a current vaccination card. Recipients of the shot must wait in Tishman for 15 minutes after the inoculation and for 30 minutes if they have a high-risk history of anaphylaxis and other allergic reactions.
For the flu vaccine, we will be partnering with New England Nurses, Inc. to offer onsite shots Wednesday, October 12, from 10:30 a.m. to 1 p.m. and 2 p.m. to 5 p.m. in the Alumnae Hall Ballroom, and Thursday, October 27, from 10 a.m. to noon in the Library Lecture Room.
Advance registration is required for flu shots by Friday, October 7.
To book your flu shot, please follow the two-step process outlined below.
STEP 1: Please go to the New England Nurses site, and you will see a drop-down menu that says select service. Select Seasonal Flu Shot, and the date of your clinic will be in white within the calendar. Click the blue arrow to go to the month of October. Click on the available date, and 10-minute intervals available for selection will pop up.
If you complete the appointment process and include your email address, you will receive a reminder email regarding the clinic appointment. Appointments are scheduled 10 minutes apart to maintain appropriate spacing within the site of the clinic.
STEP 2: Complete this Consent and Insurance Form and bring it with you to your appointment.
Included as links are the Vaccination Information Statement, HIPAA Privacy Notice, MIIS immunization sheet, and objection form. If applicable, you can bring the completed MIIS objection form to the clinic and give it to the nurse.
The vaccine will be Afluria, manufactured by Seqirus and distributed by Seqirus USA Inc., and approved and released by the FDA. It is an inactivated vaccine indicated for active immunization for the prevention of influenza disease caused by influenza virus subtypes A and type B contained in the vaccine. Additional information can be found on the CDC website.
